Nursing Assistant Job Description:

1. Responsible to: Director of Nursing
2. Purpose:
   1. To provide oversight and general supervision to clients in areas of need. 
   2. To provide active treatment to clients.
   3. To provide training programs to clients in areas identified by the Interdisciplinary Team.
3. Responsibilities:
   1. Implement training programs with clients in areas identified by the Interdisciplinary Team.
   2. Provide assistant to clients in areas of need. Communicate clients’ needs to Heath Services Supervisor.
   3. Assist clients with Activities of Daily Living skills.
   4. Encourage clients’ interpersonal skills.
   5. Monitor and supervise clients to ensure safety and well-being.
   6. Provide interventions for client maladaptive behaviors.
   7. Participate with clients in their daily routine. Follow shift schedules.
   8. Assist clients to medical, dental, and other scheduled appointments as assigned.
   9. Ability to provide physical support to residents if they are unsteady or falling.
   10. Ability to provide physical management procedures in conjunction with behavior programs as trained and assigned.
   11. Complete special projects as assigned.
4. Documentation:
   1. Maintain accurate documentation of training programs.
   2. Maintain accurate attendance records.
5. Qualifications:
   1. A minimum of 8 years of elementary school or proof of equivalent knowledge.
   2. Written communication skills necessary for record-keeping and program implementation; successful completion of facility certified aide training course within 120 days of employment.
   3. Some positions require a valid Illinois driver’s license and good driving record.
   4. Physical stamina and energy necessary to provide personal care to residents.
   5. Ability to run or walk a distance of approximately 25 yards to assist with resident emergencies or for building evacuations.
   6. Freedom from communicable diseases.
6. Exposure to Environmental Conditions:
   1. Frequent exposure to cleaning chemicals.
   2. Occasional exposure to industrial adhesives due to construction/remodeling.
   3. Frequent exposure to vapors from paint fumes (including oil based paints and stains.
   4. Occasional exposure to dust due to construction/remodeling.
